How do I create a custom pet in the sims 3 pets expansion pack?

You can add one when you initially create a household at the beginning, but that’s not what you mean here I’m sure.
Go to edit town, create a household, make a cat however you like. It should say something about not being able to play as just pets with no sims. Click past it. Click the cat from the edit town menu and click place copy. Place it in your active household. Then click yes.
Or be lazy. Adopt ANY random cat. Open up ctrl+shift+c and type testingcheatsenabled true. Then shift+click on the cat and click edit sim in create a sim.
